Died of injuries while training, as an instructor he was demonstrating with a live 68 grenade, when it fired prematurely. North Africa.

I think there might be some incorrect data here. Sergeant Sims died in Algeria and is buried there.
Serjeant Richard James Sims, of the 1 Bomb Disposal Group, Royal Engineers, was serving in Tunisia when a Mk3 68 Grenade exploded prematurely during training, severely wounding him. He succumbed to his injuries on 3 June 1943 at the age of 27.
Born in Stoke-on-Trent to Fletcher and Leonora Sims, Richard joined the Royal Engineers in 1934. He married Nora Beulah Bridges in 1937 at Cliffe Church, Kent, after meeting her while stationed nearby. The couple lived with their two young daughters at 6 Longford Place in Cliffe, where Nora had been raised.
Serjeant Sims is buried in El Alia Cemetery, Algeria, and is commemorated on the Cliffe War Memorial in the churchyard of St Helen’s Church.
